If EFI boot, then I suspect this is shim fallback.The host does not have EFI boot entry for the disk so at first boot, it boots from the removable media path EFI/BOOT/BOOT{ARCH}.EFI. If shim is installed in this location, shim fallback scans EFI/*/BOOT{ARCH}.CSV and populates EFI boot entries, then triggers a reboot.
