You could also try the following:
cout << "<form method=\"podt\" action=\"something/cgi-bin/script\">
cout << "<input type=\"checkbox\" name=\"restart_bios_config\" value=\"on\">";
cout << "</form>";
Its a little less processing and not as complex.
