I'm never sure if my posts are getting to this list, so my apologies
if this appears twice.

Here's the latest & greatest (which I think is unchanged from my last
post).  Alas, I have not filled in the paperwork, and can't remember
what I'm supposed to do.  Is there a standard form, or what?
