Second attempt.

Changes for this revision:

+ Added NEWS and ChangeLog items.
* Throw error diagnostic when encountering the ambiguous construct in
  compatibility mode (but still accept and handle it the traditional
  way).
* Add migration guidance to Texinfo manual.

It feels to me like this changeset bends over backwards to accommodate
those who might be shocked by the change (to the point that they blow
chunks all over Wales?).

Thoughts?  +1s?  NACKs?
