with 'groff -mm', it does exactly what I think you want. Sorry, I hate the default spacing around the equal sign so I always add one either side! I do not think I am doing anything special.

Doug, I have attached the PDF which should reach you but I am unsure as to whether that makes it through to the mailing list.
