Maybe the following macro helps:

  .de XX
  .  while \\n[.$] \{\
  .    nop \\$1
  .    shift
  .  \}
  ..

For example, the line

  .XX \*([X

gets expanded as

  First
  paragraph
  of
  annotation.
  .LP
  Second
  paragraph
  of annotation.

I'm not a user of refer, so I can't give more specific help.
