Look at line with %%DocumentMedia, page size is in points.

Try to pass the papersize option directly to grops on cmd line, you can
use the -P option of groff, say:

echo "Something of something" | groff -P -p -P letter | less

you should see that %%DocumentMedia is 612 792 (letter size)
