[Groff] collapsing doubled single quotes

I see that in devutf8 ` becomes ‘ and ' becomed ’.

Several man pages use ``foo'', which becomes ‘‘foo’’.

I’d prefer to see “foo” for those.

I tried changing devutf8/R et al to add:

``      24      0       0x201C
''      24      0       0x201D

based on the other entries there, but that of course didn't do it.

Essentially `` should be treated as lq and '' as rq.

Can it be done w/o patching the src or the manpages?
