[Groff] UTP -- space before box, ch06 formatting errors

Yes, some space between the top of the box and the previous line is
called for, I think. How about one line height? This should, IMO, be
built into the SS macro.

With groff v1.15 and the latest utp macros, I'm getting exactly the same
errors
with the new ch06.
