Thanks for testing out the files.  From scanning ghostscript's bug
database, it seems that pdfwrite has had a few problems with font handling
in the 7.x release branch.  One registered bug is very similar to the
ours; look at bug # 524292 at:

The ligatures in this sample pdf file (produced with TeX -> dvips ->
ps2pdf) show exactly the same bad kerning.  The bug reporter notes that
the ps file looks fine, and the bad pdf is produced only by 7.x, not by
6.5x.  The bad ligature kernings arise after a change of font size.
Could this be the reason that the error shows up on the *second* page of
my sample - i.e., due to font changes for headers/footers during the page
transition?

So, definitely a ghostscript bug - I'll send a note to the gs team.
