Strange.  I've done the same thing with the two
given files and I cannot reproduce either of the
two described behaviors.  It works just as it is
supposed to.
I'm using GNU groff version 1.17.1 and
Aladdin Ghostscript 6.0 (2000-02-03).

(The EPS file is missing a %%BeginProlog, but I can't
believe this is causing the error, since it is only
a comment and therefore ignored by the printer anyway.
Also, the "%Pages: 1 1" should perhaps be "%%Page: 1 1",
and a "%%Pages: 1" in the comments section would also be
nice (at least for version 3 of the DSC; don't know
about earlier versions), but the same also applies here.)

Could you perhaps try executing the resulting PostScript
file just with ghostscript ("gs file.ps"), and see
whether gs makes any helpful remarks?
