I don't understand the question -- Unix nroff doesn't use the ditroff
language, only troff does; and troff interprets .ul and .cu identically
as a request for italics.  Ah, you're thinking about how to implement it
in groff!  One approach would be to never underline italic spaces (which
is what currently happens, right?) but to translate space to \(ul when
in .cu mode.
