I found the following historical remarks on the roff pre-history in
S.R.Bourne's `The UNIX System', 1st ed.

> J.E. Saltzer had written "runoff" for CTSS.

CTSS was an operating system, it might be even older than Multics.  The
acronym stands for `Compatible Time-Sharing System'.  It is documented
in

Crisman, P.A. (Ed.), 1965. The Compatible Time-Sharing System.
MIT Press, Cambridge, Mass.

Moreover, troff was pronounced like `tee-roff'.
