    doc/meref.me.in: Fix content and style nits.
    
    Fix two issues raised by Dave Kemper:
    * Recast advice about document organization to cure grammar error.
    * Restore motivation for `ep` macro's existence.
    
    Also:
    * Drop "three-part" from "titles" jargon.  We don't otherwise use the
      descriptor, nor does it matter to our presentation.
    * Say simply "titles" instead of "headers and footers" subsequently.
    * Drop duplicative man page-style cross reference.
