It depends. The "Create progress note" is available from the
EMR tree root node, issue nodes, and episode nodes. It will,
respectively, open an editor for a new problem, for the
selected-from issue, or for the selected-from episode.
It will raise an existing editor for the given problem.
In any case it will switch the right hand side of the EMR
tree from browse mode to edit mode.
The mode can be switched manually any time w/o affecting the
state of either left or right hand side by means of the
button in the upper right corner (which switches labels
according to the current mode).

No. The plugins EMR tree and Notes entry are independant of
each other.
This all happens in the EMR tree only.
(both run the same code for displaying the right hand side,
of course)
