Complete is impossible and fraught with ambiguity. The rare
cases aren't worth it anyways. They are caught be the more
general second/third level queries.

It is and always has been. The one I did not think of
was "รค" -> "e" (and th->t and v/ph -> (f|v|ph et cetera).
One needs to take care to not turn "^Thomas" into
"^(T|Th)omas" for first-level queries, however, as that would
again defeat index use.
