Can only answer this one: AFAIU this is done using the autoload
mechanism - see `package-generate-autoloads'.

In effect, the parent directory of the autoloads file is added to
`load-path' when this file is loaded - typically when activating
packages.

An analog trick should allow to add subdirectories.
