The preprocessor token is
_HAVE_PORTAUDIO_
with underscores before and after (the convention is to avoid leading
underscores in user code). What is bizarre is that the leading and trailing
underscores are missing in the comment #1 on the bug tracker though they
appear in the email that the bug tracker sent, thus: (Copied from the mailing

Follow-up Comment #1:
You wrote "with portaudio" yet the definitions in midi.c are surrounded by
#ifndef _HAVE_PORTAUDIO_
so it would seem that you are building without _HAVE_PORTAUDIO_ defined. You
don't say what sources you are using to build (presumably old ones, since you
don't have the patch already?)
Why would you want to do that?
You can attach files to the bug report, rather than using an external hosting
site.

Whatever is wrong with the bugtracker, the leading underscores are needed. But
you still didn't say what sources you are using (I don't know what "ebuild" is
either ...)
