That's the behaviour I see when I use the PortAudio and ALSA MIDI backends 
(which is the combination I actually use when I'm not running JACK).
Using PortMidi (with the return statement commented out) playback doesn't 
even start properly.
I'm not yet sure what exactly is going on there. In theory only one 
backend should advance the playback time (and thus the playhead).
Usually that would be done by the audio backend (which is synchronized to 
the sound card's sample clock), while the MIDI backend just follows 
whatever the audio backend does.
