[Denemo-devel] MIDI inserts for score, movement and staff

I have put in the stuff to allow you to insert MIDI messages on a score,
movement and staff wide basis.
A funny example:

(d-DirectivePut-staff-midibytes "my" "0x90 0x44 0x60")

creates a drone on the staff (by doing a note on for the whole staff).

No use if someone plays the note and turns it off!

More serious applications - channel selection?, default volume?
