At the moment the MIDI filters expect NOTEON velocity=0 for NOTEOFF,
which is what my machine does. I think we should pre-filter our input to
give NOTEON (with velocity, say, 128) for those devices (like my M-Audio
Keystation) that generate NOTEON velocity=0 when a key is released.
I will not be able to test this though. Anybody have a MIDI controller
that generates NOTEOFF?
