* list.h (struct list) [! NDEBUG]: Add field name.
(list_init): Add parameter name. Update users.
(LIST_CLASS): Update template to account for list_init changes.
(list_unlink): Improve debugging output.
* activity.c (activity_prepare): Explicitly initialize all
lists.
