Minor enhancements to form_driver_w examples

Hi,

I've made some minor enhancements to the form_driver_w example so that
only the new routine is used, and the old form_driver is avoided.

Patch 1: Cleanup inconsistent indentation
Patch 2: Always use form_driver_w
