Details:

Many of the various FETCH-MESSAGE-* procedures call WITH-FOLDER-LOCKED and
then interact with the server, which may reply with responses whose processing
causes other operations on the folder's cache that require its lock.  Either
WITH-FOLDER-LOCKED should defer these operations on the folder, or it should
identify recursive uses.
