The fact that I still don't understand this aspect of lilypond is no doubt a
bug in me, but I'm pretty sure that the example in this section of the NR has
its own problem: I cannot see any material difference between the two bars of
the output, but the text below it refers to collisions between noteheads and
accidentals in the first measure.
Compare the output in the 2.19 and 2.16 NR:
