These are set when the translator starts a new .po file. When she uses
msginit for this purpose, as described in the gettext documentation,
all the fields will be filled automatically. Otherwise, depending on
the translation editor, some of the fields may need to be filled in
by hand.

In any case, these fields are supposed to be edited by the translator.
The template therefore contains only the placeholders.

Yes. But since the VERSION on the same line is modifiable by the translator,
let's simply treat PACKAGE the same way.
