Thanks for the report and test files.  The problem was due to the
output routine now being unmodified by default.  If the output routine
got called before the first preview, it erased the \AtBeginDvi
initialization, stopping psfrag (and possibly other stuff) from
working.

I checked in a change.  If you experience any other problems, please
report them.  The output routine change is a rather drastic one, so it
would be nice to get more testing before the next release.
