I tried to use timeout=0 like:
dial timeout=0 %phone_number maxTime=20
still can't hear the first 2 rings.

Another option is to try is unsupervised (dtmf) dialing. I think that
is with dial.dtmf, or dial timeout=0. The join will then start
immediately after the dial completes rather than waiting on answer
supervision. The other side would then hear the ringback as sent from
the public network.
