Follow-up Comment #1:
I think your solution is the correct one: use the buffer signal. This
one is supposed to be active all the time while AVRDUDE has the ISP
connection open.
The program LED is really only meant to be active while AVRDUDE is
writing to the AVR. Changing its meaning now might alarm other users
who are used to the current behaviour, making them believe they might
change something in their AVR inadvertently when they are actually
only reading.
