Hi,

More dox, hot off the grill!

I've done most of the work for merging Ch 2 of Rich Neswold's document.

This one was a bit more intrusive than normal, so I'd like some feed back
before I commit it. If anyone has some free time, please try the patch and
let the list know if I broke the build. Also, check 'make dist' and verify
that no files are missing.

I'm gonna sit on this one for few days.
