[avr-libc-dev] <avr/sfr_defs.h> documented

Although this file is not normally included directly by application
programs (only indirectly through <avr/io*.h>), i took the occasion
to also explain the memory-mapped IO paradigm vs. using a specific
IO address space (and in/out instructions).  All this is written
from a C programmer's perspective.

Please have a look at it (already in CVS), and correct me where
i'm mistaken.
