Formatting cleanups in macro comments.

While looking at fortran.m4, I noted a few underlines not aligned with
the macro synopsis above.  Not a big deal, but they distracted me, so
let's just fix them all.  The quickfix list generated by

  for f in `git ls-files`
  do
    awk '{ len[NR] = length($0) }
         /----*/ && len[NR-1] != 0 {
           if (len[NR-1] != len[NR])
             print FILENAME ":" NR ":" $0
         }' $f
  done

makes them easy enough to find, as well as a couple of missing commas
in the macro synopsis.  I'll fix the couple of offenders from files
imported from Automake there in a moment.
