It only causes trouble if the format sets _ to a catcode and/or
definition that does not expand to a character token. The xmltex
format in its current version has this problem, and we had a major
messup at my work place because of this just today.
I actually have no idea how this is supposed to play together with
%&format
comment lines.
With normal plain TeX, LaTeX or iniTeX settings, _ is safe to use in
startup filenames, as opposed to ~.
Using something like underscore.sty, an \input in the middle of the
document might cause trouble again.
