Here is a report which clearly points to a misconstructed
Speedstream 5660, its NAPT table is too small:

There is even a Windows app for resetting the NAPT table;-)

Hm, there seems to be lots of routers of there who can't handle P2P-related
requests - not only MLDonkey is suffering from this. The only solution is to
lower max_opened_connections to 150 or even less, also
max_connections_per_second should be lowered. Consider using a genuine Linux
router (IPcop, fli4l etc.)

Late, but may be useful for others...
I have the same problem with a Speedstream 5660. Even if I limit the bandwiDth,
it KILLS my connection.
Why? Because it uses all the NAPT table. If I telnet to the router and I
"#clear naptmap", everything starts working... Until it fills the table again!
Another way to do so is to restart the router, but it takes longer ;)
I have done a little script to this using EXPECT.
My old 3COM 812 didn't have this annoyance, but a storm burnt it...
Limit max_opened_connections. It helps, but it doesn't stop blocking.
BTW: QOS DOESN'T HELP AT ALL.

Try limiting traffic with QOS.

I have the same problem. I have a 128/512 ADSL. The problem is actually that
after a while it'll get harder and harder to open connections, until it becomes
impossible. max_opened_connections is the default 200 for me.

What value has max_opened_connections?
