    doc/groff.texi: Fix content and style nits.
    
    Content:
    * Add program index entry for "vtroff", since we mention it.  Existing
      practice seems to be to use this index for any program (or file)
      mentioned, not just those that groff distributes.
    
    Style:
    * Adjust dead-tree pagination.
    * Vary wording.
