At least we've now got proof that there's something wrong
with the code filling the inbox with items:

Can you provide the following information:
- when this happens, does the inbox show all items or only
those for the active patient ?
- when you say "sign document from the inbox" you mean "double-click
the inbox item saying that there's new documents" (which takes
you to the document plugin), right ?
- when you double-click the inbox item, did you already have
an active patient ?
- if so, was it the patient referenced from the inbox item or a different
patient ?
Oh, and there's another hint:

Inbox items shouldn't be lists ...
I'll investigate. Please help with providing the above
information so I can better reproduce the situation.
